Avenue 360 Health & Wellness is a comprehensive and designed to provide integrated care for children, adults and families living in an economically disadvantaged and underserved communities. The centers provide access to comprehensive primary care, behavioral health, dental care, prevention and health education, and community health services. Healthcare services are provided by a multidisciplinary care team of healthcare professionals that include physicians, nurse practitioners, registered nurses, pharmacists, social workers, dietary and nutrition professionals, behavioral health providers, and dentists. Avenue 360’s care team works to meet the individual healthcare needs of each patient utilizing a personalized health plan. Avenue 360 also offers a variety of community-based services aimed at creating healthy and safe environments, reducing health disparities, and encouraging patients to make informed decisions. These services include but are not limited to community health education, health screenings, disease prevention initiatives, nutrition counseling, and mental health services.
